When you book your first appointment, you will receive a comprehensive, 14-page medical history questionnaire. You will be required to complete this form and submit it no less than one week before your visit. Prior to your appointment, Dr. Saxena will thoroughly review your medical history in preparation for your consultation. We ask that you submit your payment at the time of booking your appointment since a significant amount of time is spent by the doctor to review and prepare in advance of your first visit.

During your initial visits, the doctor will review your history, as well as:

• Identify your goals and expectations
• Listen to your health-related story and determine the nature of your current medical challenges and coping resources
• Provide education regarding suspected or confirmed disease processes
• Decide whether any further diagnostic evaluation is needed
• Develop a plan of action toward your ideal health goals
