If you are new to the practice, please download and print the following form.  Please take the time to fully address each section of the form.  Once you have completed the form please fax to our office at 813.269.2701.  Upon receipt and review of your forms and verification of your LWU registration, you will be contacted by our New Patient Coordinator to schedule your first appointment.
